Susquehanna Ave., Philadelphia, PA 19125 - The employer did not ensure that an engineering survey was performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure before employees conducted demolition operations on second and third floors of a residential building, thus exposing the employees to struck-by and fall hazards.  This condition was observed on or about June 20, 2018.  Espana Brothers Corporation was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 978940, citation number 1, item number 2, group A, and was affirmed as a final order on December 18, 2014 with respect to a workplace located at 4100 Parkside Avenue, The Lansdowne Building, Philadelphia, PA 19104.
