Lincoln Recycling, Inc., 986 French Street, Meadville, PA  16335, main building / warehouse, on or about June 4, 2014 and times prior thereto: Employees were exposed to struck-by hazards from the potential fragmentation of defective/improperly maintained grinding wheels.  The left side of a Central Machinery 8 Bench Grinder (Item #39798) was equipped with a stone grinding wheel.  The left side of the stone wheel was curved, and the entire wheel was not properly dressed to ensure that the entire contact surface of the wheel was flat.
