29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: A.) Lincoln Recycling, Inc., 986 French Street, Meadville, PA 16335, main building / warehouse, on or about June 4, 2014 and times prior thereto: The Tongue Guards on the right and left sides of the Central Machinery 8 Bench Grinder (Item #39798) were not adjusted properly. The tongue guards were over a 1-inch from the grinding wheels. The bench grinder is used for work operations that include, but are not limited to, grinding tarnish off of metal to identify the type of metal.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: A.) Lincoln Recycling, Inc., 986 French Street, Meadville, PA 16335, main building / warehouse, on or about June 4, 2014 and times prior thereto: A Central Machinery 8 Bench Grinder (Item #39798) equipped with adjustable work rests. The work rests on the right and left sides of the bench grinder were adjusted to 3/8-inch and 5/8-inch, respectively. The bench grinder is used for work operations that include, but are not limited to, grinding tarnish off of metal to identify the type of metal.

OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: A.) Lincoln Recycling, Inc., 986 French Street, Meadville, PA 16335, main building / warehouse, on or about June 4, 2014 and times prior thereto: Employees were exposed to struck-by hazards from the potential fragmentation of defective/improperly maintained grinding wheels. The left side of a Central Machinery 8 Bench Grinder (Item #39798) was equipped with a stone grinding wheel. The left side of the stone wheel was curved, and the entire wheel was not properly dressed to ensure that the entire contact surface of the wheel was flat.
