OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from fall hazards: a.) Lincoln Recycling, Inc; - On or about 10/2/24, and at times prior thereto, employees were being lifted on pallets with the forklift (Doosan Model #GS5E-7). The employees were lifted 6-10 feet to unlatch the bin on the lugger trucks. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) Lincoln Recycling, Inc. - On or about 10/15/24, and at times prior thereto, employees were holding pin extenders while hydraulics actuated the pin. Employees hands were at the point of operation or within 3 inches. An amputation occurred while an employee was holding the pin extender. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Items stored in front of the panel boxes, included, but were not limited to, plastic buckets, basketball, and a cardboard box.
