29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).    On or about 5/28/2014, at Flour Line 4, employees cleaning the ovens did not isolate the natural gas energy coming to the ovens and instead depended on electrical equipment isolation as a means of protection.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(5)(ii): Lockout devices and tagout devices were not used for lockout and tagout purposes only and were not singularly identified.    On 5/28/2014, the employer did not ensure locks used for lockout purposes and were singularly identified. Instances included:    1 - In the Corn Cook Room, a lock intended for lockout purposes was used on a pneumatic dumper for the corn conveyor system to place the equipment out of service so it would not be used and the lock was not identified.    2 - In the production room, Authorized employees cleaning Flour Line 4 ovens had not identified locks placed on the line for energy control during maintenance activities.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.    Ole Mexican Foods, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(5)(ii), which was contained in OSHA inspection number 911782, citation number 3, item number 1 and was affirmed as a final order on 1/7/2014, with respect to a workplace located at 6585 Crescent Drive, Norcross, Georgia.
