29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: On or about June 6, 2014, D. Construction, Inc. failed to document the instructions that it provided its employees regarding the hazards associated with failing to properly secure the chipper hood of a Brush Bandit Brush Chipper Model 280 (D. Construction, Unit #294).
