29 CFR 1910.146(c)(7)(iv): When hazards arose within a permit space that had been declassified to a non-permit space under 29 CFR 1910.146(c)(7), each employee in the space did not exit the space.  The employer did not reevaluate the non-permit space and determine whether it must be reclassified as a permit space in accordance with other applicable provisions of 29 CFR 1910.146(c)(7):  As required by the above referenced standard and 29 CFR 1960.8(b): On or about June 7, 2014, Vault B-485, Bowling Alley:  The employer did not re-evaluate a non-permit required confined space when a hazard was introduced into the area which affected the atmosphere.

29 CFR 1910.269(a)(3): The employer did not determine existing conditions related to the safety of the work to be performed before work on or near electric lines or equipment was started:  As required by the above referenced standard and 29 CFR 1960.8(b):  On or about June 12, 2014, Main Substation #463:  High voltage electricians were provided with switching orders, which, if executed, would have caused serious physical harm to employees and/or equipment.  The switching orders were written without the employer determining existing conditions related to the safety of the work to be performed.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used:  As required by the above referenced standard and 29 CFR 1960.8(b):  On or about June 7, 2014, Vault B-485, Bowling Center:  Employees using chemicals, including Medium Aliphatic Petroleum Solvent and Citrus Terpenes, had not been informed of steps to take to prevent exposure in varying environmental conditions.  Employees were using the Type HP and Type GP cleaner/degreaser in an area with limited ventilation.
