29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm in the employees were exposed to fire and explosion hazards: Power Plant: On, or about, December 8, 2016, Boiler 1S, a Cleaver-Brooks fire tube boiler, experienced a malfunction that caused an explosion in the burner section, which in turn blew the front open and caused damage to the internal components. The boiler had been experiencing persistent operational issues since it was commissioned. Although efforts were being made to correct the issues with Boiler 1S, it remained in service running on natural gas. Boiler 1S did not have any reported issues when running on diesel fuel/#2 oil.
