Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:        a) On or about June 19, 2014, at a jobsite located at 1541 West Fry Street, Chicago, IL. - employees performing masonry activities were working near unprotected sides and edges without the use of fall hazards protection, thereby exposing the employees to fall of approximately fifteen (15) feet three (3) inches.    The VH Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(14) which was contained in OSHA inspection number 313933129, citation number 1, item number 1 and was affirmed as a final order on May 5, 2012, with respect to a workplace located at 1542 W. Chestnut Street, Chicago, IL 60642.
