29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes   (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a) On or about March 23, 2016, at the above addressed jobsite - employees performing masonry activities on the second floor were exposed to falls created by the presence of an unprotected floor hole (stairway opening), thereby exposing employees to fall hazard of approximately fourteen (14') feet four (4) inches to ground level.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews:    a)  On or about March 23, 2016, at the above addressed jobsite - employees performing masonry operations on a tubular welded framed scaffold were not protected from falling due to missing end rails along the scaffold, thereby exposing the employees to fall hazards of over fourteen (14) feet four (4) inches.    The VH Masonry Construction, Co. was previously cited for a violation of this occupational safety and health standard or   its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 912707, citation number 1, item  number 3 and was affirmed as a final order on October 29, 2013, with respect to a workplace located at 1707 N. Whipple Street, Chicago, IL. 60647.
