29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) For employees working at 5226 Admiral Pointe Drive in Apollo Beach, as observed on or about June 24, 2014. Employees were observed installing plywood sheathing on a roof with a 6:12 pitch. The eave to ground height was approximately 21 feet and the employees had no fall protection system in place.    b) For employees working at 5226 Admiral Pointe Drive in Apollo Beach, as observed on or about June 24, 2014. Employees were observed working on the second floor of the new residential home with an approximate 8 foot long by 4 feet 6 inch wide opening in the floor. Employees were exposed to an approximate 9 foot 4 inch fall hazard to the lower level and the employees had no fall protection system in place.    c) For employees working at 5226 Admiral Pointe Drive in Apollo Beach, as observed on or about June 24, 2014. Employees were working on the second floor of the new residential home with 9 unguarded window openings. Floor to window sill height measured  approximately 33 5/16 inches tall. Employees were exposed to a fall height of approximately 13 feet.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder.    a) For employees working at 5226 Admiral Pointe Drive in Apollo Beach, as observed on or about June 24, 2014. Employees were observed using a step ladder to access and exit the second floor. The height to the second floor landing measured approximately 9 feet 4 inches and the employees were using an 8 foot step ladder for access.

Employees were observed using the top step of an 8 foot step ladder. The employees were stepping off the top step to get onto the wall framing.

Employees were observed using an extension to supply power to electric power tools that did not have a continuous path to ground. The extension cord was missing the ground prong.
