At the jobsite, the snap hooks on the lifelines employees were using while working on a 6/12 pitched roof were secured back to nylon webbing of the lifelines themselves, exposing the employees to a 10 foot fall hazard, on or about 9/24/2015.

At the jobsite, the lifelines employees were using while installing plywood sheeting on a 6/12 pitched roof were secured in place around a single 2x4 wooden board in the roof trusses, exposing the employees to a 10 foot fall hazard, on or about 9/24/2015.

Davis Street, Tampa, FL.  All Coast Contractors, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1016795, citation number 1, item number 4 and was affirmed as a final order on 1/29/2015, with respect to a workplace located at 425 Island Cay Way, Apollo Beach, FL.  All Coast Contractors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 982322, citation number 1, item number 1 and was affirmed as a final order on 9/5/2014, with respect to a workplace located at 5226 Admiral Pointe Drive, Apollo Beach, FL.
