29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: The employer failed to conduct a periodic inspection of the energy control procedures including but not limited to the Goss Sunday 2000 printing press #20. Employees were exposed to crush-by hazards while performing maintenance and servicing activities. Abatement documentation is not required for this item. Abatement was delivered on 10-31-14.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): The employer failed to apply procedures for the control of potentially hazardous energy when employees were webbing up the Goss Sunday 2000 printing press, exposing employees to crush-by hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
