29 CFR  1910.28(b)(1)(i): The employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:  (A) Guardrail systems; (B) Safety net systems; or (C) Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems.  On or about January 30, 2026, the employer did not ensure that employees performing work tasks such as, cleaning blanket rollers from the platforms between the color stations on Heidelberg Harris printing press #15, were protected from falling over 4 feet to ground below likely to result in serious injury.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  (a) On or about January 30, 2026, the employer did not ensure that the oscillating form roller's end-protrusion, at the color stations of the Heidelberg Harris printing press #15, was guarded to prevent employee's exposure to a caught-in hazard when performing work tasks in the area such as, cleaning blanket rollers.   (b) On or about January 30, 2026, the employer did not ensure that the in-going nip points of the running rollers, at the color stations of the Heidelberg Harris printing press #15, were adequately guarded to prevent employee's exposure to a caught-in hazard when performing work tasks in the area such as, cleaning blanket rollers.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  On or about January 30, 2026, the employer did not ensure that an energized SL Waber temporary power tap was not used as a substitute for fixed wiring of a local-branch circuit receptacle (e.g. electrical outlet box) and was not zip-tied to an electrical conduit running on the vertical beam. Employees working in the shipping and receiving area were exposed to hazards associated with unsafe use and installation of the relocatable power tap energizing the camera buzzer system.   There is no abatement certification or documentation required for this item.
