29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On or about 6/26/14, at the Baldwin Georgia facility, the employer had not implemented a written Lockout/Tagout program with specific written procedures for equipment employees were performing service and maintenance to at the facility including ovens, presses, and saws.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s).  On 6/26/14, employees were exposed to cut/amputation hazards as the Accessory Saw in the production area did not have an adequate guard in that when the saw blade rises up from the table (below) to cut the product, the guard does not cover the exposed blade that is not covered by the product being cut. The guard opening at the saw measures 10 ½ x 3 ¾.   No abatement certification or documentation is required for this item.

On or about 6/26/14, in the production area, a muti-strip receptacle was being used to power a temporary electrical extension cord that was powering heat gun used in the glass area. The multi-strip receptacle had a plastic housing and was not intended for that purpose.    No abatement certification or documentation is required for this item.

On or about 6/26/14, at the Baldwin facility, the employer did not ensure orientation training for newly hired employees met the OSHA requirements. Chemicals used at the facility include: Glass Coating, Polyvinyl Chloride Compound, Isopropyl Alcohol and Aluminum Alloy.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
