29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) The employer failed to ensure that each employee conducting roofing work at 7026 West Brentwood Avenue in Milwaukee, Wisconsin, were protected from falling to the lower level by the use of a fall protection system.  This exposed employees to fall hazards of approximately 12.5 feet (ground to peak height) and a 9.4 feet (ground to eve).

This exposed employees to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) The employer did not ensure that the ladder side rails extended at least 3 feet above the roof eve.  This exposed employees to a fall hazards of approximately ~9.4 feet (peak) and ~12.5 (eve) feet.
