29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) Front porch roof, new townhome construction located at 436 Field View Lane, Milton, GA 30004: Employees were exposed to falls of approximately 14-feet, 5-inches while using ladders which were not extended at least 3-feet past the eave or secured to the structure to prevent accidental movement during use.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Porch roof, new townhome construction located at 436 Field View Lane, Milton, GA 30004. Employees were exposed to falls while working on a 4/12 pitched metal roof at eave heights of approximately 14-feet, 5-inches with no fall protection system in place. Woodman Insulation Co. Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 898342, citation number 1, item number 1 and was affirmed as a final order on 05/22/2013, with respect to a workplace located at 4227 Hardy Avenue, Smyrna, GA 30080.
