29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 1 July 2014, and at times prior thereto, three workers performing masonry work were not utilizing head protection, exposing the workers to a struck by hazard from falling materials, tools, and debris, in addition to overhead objects.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about 1 July 2014, and at times prior thereto, three workers performing masonry work on a tubular-welded frame scaffold used the components of the scaffold to access the second and third tier, exposing the workers to a fall hazard of approximately 15 feet 1 inch or 22 feet 7 inches to the ground below.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below.  On or about 1 July 2014, and at times prior thereto, three workers performing masonry work on a tubular-welded frame scaffold did not utilize toeboards as a fourth mason was on the ground providing materials for them above, exposing the worker on the ground to the hazard of being struck by falling materials, tools, or debris from approximately 15 feet above.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about 1 July 2014, and at times prior thereto, two workers were performing masonry work on a partial roof at the top of the first floor without the use of fall protection, exposing the workers to a fall hazard of approximately 15 feet to the ground below.
