29 CFR 1926.451(b)(1)(i): Each platform unit was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide: On or about 9 March 2017, employees were working on a tubular welded frame scaffold that was approximately 30 feet in height, and not fully planked, exposing them to a fall hazard.

29 CFR 1926.451(h)(2)(ii): Toeboards were not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below: On or about 9 March 2017, employees were working on the first and second tiers of a tubular welded frame scaffold that was approximately 30 feet in height, that did not have toe boards, exposing them to the hazard of being struck by falling material.
