29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. On or about 21 June 2017, two employees were laying brick on a residential building under construction while standing on the top tier of a two-tier scaffold up to approximately 14 feet high that was missing guardrails on the ends of the scaffolding, exposing the employees to a fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, personnel hoist, or similar surface was not used. End frames were used as a means of access and egress: On or about 21 June 2017, two employees were using the end frames to access a two-story scaffold up to approximately 14 feet high while installing stucco wire onto a residential building under construction, exposing the employees to a fall hazard. Stucco USA was previously cited for a Repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 1074052, citation number 1, item number 1, and was affirmed as a final order on 14 March 2017 , with respect to a workplace located at 3049 Sachen Street, Georgetown, TX 78626. Stucco USA was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 983079, citation number 1, item number 2 and was affirmed as a final order on 27 August 2014, with respect to a workplace located at 112 Bastrop Drive, Georgetown, TX.
