29 CFR 1917.151(b)(1): Danger zones on machines and equipment used by employees were not guarded: Maintenance shop: On or about July 2, 2014 and times prior thereto, the backside of the heater fan was not guarded and was within a few feet of where employees would get bolts and nuts out of bins. This exposed employees to potential lacerations hazards to their hands.

29 CFR 1917.151(g)(1): The safety guard or the tongue was not adjusted so that they were always close to the periphery of the wheel: Maintenance shop: On or about July 2, 2014 and times prior thereto, the Dayton power grinder tongue guard measured at 3/4 of an inch. This exposed employees to a potential struck-by hazard if the wheel broke during use.

29 CFR 1917.122(b): The employer did not post directional signs indicating exit routes where the exit was not visible from the employees' work stations. Conveyor tunnels: On or about July 2, 2014 and times prior thereto, it was not clear in the grain tunnels as to the direction of travel to the nearest exit. This exposed employees to smoke inhalation hazards from delays in getting to the nearest exit.
