29 CFR 1926.451(c)(3): Supported scaffold poles, legs, posts, frames, and uprights shall be plumb and braced to prevent swaying and displacement.    A) Pump jack scaffold, back of house- Employees were exposed to fall hazards of approximately 12.6 feet while working on a pump jack scaffold that was not braced at the bottom, on or about January 17th, 2014.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.   A) Pump jack scaffold, back of house- An employee and the scaffold work platform fell approximately 12.6 feet as it was not inspected for visible defects by a competent person, on or about January 17th, 2014.

29 CFR 1926.451(g)(1):   Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   A) Pump jack scaffold, rear of house - Employees were exposed to fall hazards of approximately 12.6 feet  while working on a pump jack scaffold that was not protected by a complete guardrail system (including end rails) or other means of fall protection in place, on or about January 17th, 2014.   Frame Q LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 897001, citation number 2, item number 1 and was affirmed as a final order on April 28th 2014, with respect to a workplace located at 31 New St. In Englewood Cliffs, NJ.
