The employer did not determine and implement administrative or engineering controls to achieve compliance with the permissible exposure limit, on or about June 24, 2014.  ABATEMENT NOTE:   GENERAL METHODS OF CONTROL APPLICATION IN THESE CIRCUMSTANCES INCLUDE, BUT ARE NOT LIMITED TO THE FOLLOWING:   1. Ensure the gas powered high pressure washer stays outside the building and not used in the same room in which the  power spraying operation is in process.  2. Ensure employees are trained on the signs and symptoms of exposure to carbon monoxide. 3. Install a backup pump that can be used when the original pump malfunctions.

A sanitation employee was exposed to carbon monoxide at a time weighted average of 528 parts per million (ppm), approximately 10.57 times the permissible exposure limit (PEL) of 50ppm, on or about June 24, 2014.     MVP KOSHER FOODS, LLC A/K/A MEHADRIN KOSHER POULTRY, LLC AND ITS SUCCESSORS BIRDSBORO KOSHER FARMS, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION STANDARD OR IT'S EQUIVALENT STANDARD  1910.1200(h)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315887638, CITATION NUMBER 1, ITEM NUMBER 14(b), AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 10, 2013, WITH RESPECT TO A WORKPLACE LOCATED AT 1100 LINCOLN ROAD BIRDSBORO, PENNSYLVANIA.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
