29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) On or about 7/10/14 at a roof replacement project located at 210 W Main Street, Springville, New York; employees were using a ladder jack scaffold while replacing a roof on a 3 story building. The scaffold was approximately 16 feet high and the employees were not wearing a personal fall arrest system.    NO ABATEMENT CERTIFICATION REQUIRED      Switala's Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1)(i), which was contained in OSHA inspection number 314717042, citation number 1, item number 2 and was affirmed as a final order on 4/18/10, with respect to a workplace located at 6188 Sheridan Drive, Buffalo, New York.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) On or about 7/10/14 at a roof replacement project located at 210 W Main Street, Springville, New York; employees were tearing off old roofing on a 3 story building that had a pitch of 6 in 12 and an eave height of approximately 30 feet. There was no fall protection in place.    NO ABATEMENT CERTIFICATION REQUIRED    Switala's Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 314717042, citation number 1, item number 3 and was affirmed as a final order on 4/18/10, with respect to a workplace located at 6188 Sheridan Drive, Buffalo, New York.
