29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Adena Corporation on or about July 15, 2014, at the address of 715 Richland Mall, Mansfield, OH: The employer failed to ensure employees performing flatwork on the 2nd story near an unguarded mechanical opening were adequately protected from falls. The employees working in this condition was exposed to a near16 ft. fall to the ground below.
