29 CFR 1926.1060(b): The employer did not provide retraining as necessary so that each employee exposed to fall hazards maintained the understanding and knowledge acquired through initial training: On or about 8/20/14, the employer did not ensure employees were provided re-training in the recognition and avoidance of unsafe conditions while using ladders. Two employees for Adena Corporation used an A-frame step ladder inappropriately causing an employee to fall while accessing a scaffold.
