Stucki Company, Inc. dba The Hill Railroad Car Company, 201 Power Street, New Castle, PA  16102, on or about July 15, 2014 and times prior thereto:  Employees utilize a Yale Forklift (M/N:  GTP080LGNGBE088, S/N:  B813D01778T) to perform functions including, but not limited to, moving sheet metal.  The backup alarm for the forklift was not functioning.    B.) A. Stucki Company, Inc. dba The Hill Railroad Car Company, 201 Power Street, New Castle, PA  16102, on or about July 15, 2014 and times prior thereto:  Employees utilize a Yale Forklift (M/N:  GTP080LGNGBE088, S/N:  B813D01778T) to perform functions including, but not limited to, moving sheet metal.  The forklift tires had excessive wear and gouging.

Stucki Company, Inc. dba The Hill Railroad Car Company, 201 Power Street, New Castle, PA  16102, on or about July 15, 2014 and times prior thereto:  Employees operate a Buffalo Iron Worker (56Z-6828) to perform tasks including, but not limited to, punching holes into sheet steel.  The guard for the punch side of the iron worker had a 3-inch by 4-inch opening in the guard.  The opening in the guard allowed for access to the point of operation.  Employees could reach trough the guard to access the point of operation.

Stucki Company, Inc. dba The Hill Railroad Car Company, 201 Power Street, New Castle, PA  16102, on or about July 15, 2014 and times prior thereto:  Employees operate a band saw to perform work duties, including, but not limited to, cutting three inch metal stock.  The adjustable guard for the band saw would not adjust below five inches in height.  The condition exposed the unused portion of the band saw blade.    B.)  A. Stucki Company, Inc. dba The Hill Railroad Car Company, 201 Power Street, New Castle, PA  16102, on or about July 15, 2014 and times prior thereto:  Employees operate a Pacific Hydraulic Press Brake (M/N:  300-14, S/N:  1792) to perform tasks including, but not limited to, bending structural grade steel.  The rear of the press brake was not guarded to prevent access to moving parts which included but were not limited to the moving ram and upper tooling.
