29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition:     On or about July 16, 2014, the floors of PIO Custom Cabinetry, Inc., were not maintained in a clean condition.  There were excessive wood dust accumulations throughout the shop floor posing a fire hazard.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.107(b)(9): A clear space of not less than 3 feet on all sides was not kept free from storage or combustible construction:     On or about July 16, 2014, PIO Custom Cabinetry, Inc., owned and operated a finishing paint spray booth.  Combustible and flammable materials, such as paint, solvents and scrap wood were stored within 3 feet of the spray booth.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:     On or about July 16, 2014, an operator conducting spray finishing operations was required to wear a half-mask, negative pressure respirator with cartridges.  The employer did not establish and implement a written respiratory protection program.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.303(c)(3)(i): Conductors were not spliced or joined with splicing devices identified for the use or by brazing, welding, or soldering with a fusible metal or alloy.   Soldered splices were not spliced or joined mechanically and electrically secured first without solder and then soldered.  All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose.      On or about July 16, 2014, conductors on a table saw feeder were spliced together.  The splice was not spliced or joined with a splicing device for the use or by brazing, welding or soldering with a fusible metal or alloy.  The splice was covered and secured with electrical tape.    Abatement certification is required of this item in accordance with the 29 CFR 1903.19(c).

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:     On or about July 16, 2014, extension cords were used as a substitute for fixed wiring at PIO Custom Cabinetry, Inc.:     a. An extension cord was used to power both the Miter Box Saw and an exhaust fan for the dust collector on the CNC lathe.     b. An extension cord was affixed to a small cabinet and was used to charge hand-held power tools.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
