29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: On or about July 18, 2014, at least five employees installing a sheet metal roof and siding at a commercial site were accessing the roof using a work platform elevated by a Genie GTH-1048/1056 Rough Terrain Forklift that was not an approved work platform by the manufacturer for capacity, construction, use, and fall protection exposing the employees to a fall hazard of more than 16 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about July 18, 2014, and times prior thereto, employees performing roofing work were using an extension ladder that did not extend (3) feet above the landing to access and egress the roof. The employees were exposed to a fall hazard of up to (16) feet to the ground below.
