On 7/16/2014, an employee from Cecil Compton Construction LLC was installing siding on a two-story residence located on Peruque Hills Court in Wentzville, MO. The employee was on ladder jack scaffolding, which was placed on the 4/12 pitch roof of the first story. He was approximately 13 feet above the ground and was not protected by a personal fall arrest system.

On 7/16/2014, an employee from Cecil Compton Construction LLC was installing siding on a two- story residence located on Peruque Hills Court in Wentzville, MO. The employee was on ladder jack scaffolding, which was placed on the 4/12 pitch roof of the first story. The top of the ladders were not secured to the gable to prevent slipping or horizontal movement.
