29 CFR 1910.1000(c): An employee(s) was exposed to silica (crystalline quartz) in excess of the 8-hour time weighted average limits of 0.408 mg/m3  listed in Table Z-3:    a) The concrete mixing area:  An employee who mixes the concrete had an exposure to silica (crystalline quartz) of 0.683 mg/m3 during the 437 minute sampling period conducted on September 16, 2014.  Zero exposure was assumed for the 43 minutes not sampled.

29 CFR 1910.1000(e): To achieve compliance with paragraphs (a) through (d) of this section, administrative or engineering controls must first be determined and implemented whenever feasible. When such controls are not feasible to achieve full compliance, protective equipment or any other protective measures shall be used to keep the exposure of employees to air contaminants within the limits prescribed in this section. Any equipment and/or technical measures used for this purpose must be approved for each particular use by a competent industrial hygienist or other technically qualified person. Whenever respirators are used, their use shall comply with 1910.134.   a) Mixing area:  The employer did not implement administrative or engineering controls to reduce the exposure  to silica for an employee who mixed the concrete and had an exposure to silica (crystalline quartz) of 0.683 mg/m3 during the 437 minute sampling period conducted on September 16, 2014.  Zero exposure was assumed for the 43 minutes not sampled.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields:    a) Welding area: Employees working or walking near the welding area were exposed to arc welding rays due to the welding area not covered by the welding screens.    Dixie Precast was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 314641184, citation number 1, item number 3 and was affirmed as a final order on July 12, 2011, with respect to a workplace located at 2950 Angellette Drive, Austell, Georgia.
