29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds used was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) a) On or about 07/21/2014 at the west corner of building #9260, the scaffold was not fully planked while the employees were applying stucco to the walls of the shopping center building. Employees were exposed to a 19 ft. fall hazard.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On or about 07/21/2014 at the west corner of building #9260, a sectional scaffold system had missing ends and mid-guardrails on all levels while employees were applying stucco to the building. Employees were exposed to a 19 ft. fall hazard.
