On or about June 24, 2015, on a 4:12 pitch gable, an employee performing lathing operations was not protected from a 9-foot 10-inch fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.

On or about June 24, 2015, an employee used a 6-foot a-frame improperly, in that the ladder was positioned on a 4:12 pitch lower roof, being used in the closed position, exposing the employee to a 9-foot 10-inch fall hazard.
