29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) Jobsite, 446 Wissinger Road, Alum Bank, PA 15521, Top Coat: On or before July 22, 2014, employees were exposed to electrical shock or electrocution hazards in that equipment used for paving operations was elevated and contacted an energized uninsulated single phase 13,200 volt power line. (b) Jobsite, 446 Wissinger Road, Alum Bank, PA 15521, Scratch/Leveling Coat: On or before July 22, 2014, employees were exposed to electrical shock hazards in that equipment used for paving operations was elevated to within approximately two feet of an energized uninsulated single phase 13,200 volt power line.

29 CFR 1926.416(a)(3): When and where energized circuits existed, the employer failed to post and maintain proper warning signs, advise employees of the location of the hazard involved, and the proper protective measures to be taken: (a) Jobsite, 446 Wissinger Road, Alum Bank, PA 15521, Top, Scratch, and Leveling Coat: On or before July 22, 2014, employees were exposed to electrical shock or electrocution hazards in that warning signs were not provided to identify and educate the employees on the hazard of the energized power lines.
