29 CFR 1926.852(e): Chute openings, into which workmen dumped debris, were not protected by substantial guardrails approximately forty-two inches above the floor or other surface on which men stood to dump material: On or about 7/23/2014 during the installation of roofing materials at a height of approximately 50 feet on a low slope roof the employer did not ensure that the trash chute was protected by a complete guardrail system approximately 42 inches above the floor.
