Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. On or about July 24, 2014, at the above addressed jobsite, employees performing bricklaying operations were exposed to falls, approximately thirty (30) above the ground, created by the presence of unprotected sides.

On or about July 24, 2014, at the above addressed jobsite, employees conducting bricklaying operations were exposed to falls due to the guardrail system around the stairway of the roof was not capable of withstanding 200 pounds of force, thereby exposing employees to falls of approximately seven (7) feet.
