29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):   A) LWCF mezzanine floor, on or about August 5, 2014, the guardrail system along the open sided floor was only 35 inches high and did not have a mid-rail, exposing employees to a fall hazard of 9 ½ feet.   B) Dump shoot on southwest end of Kiln, on or about September 24, 2014, a steel plate used as a guardrail on the side of a dumping platform was only 21 inches high, exposing employees to an 8 foot fall hazard.    C) Concrete stack, on or about September 23, 2014, there was an 18 inch opening between the guardrail system and the ladder cage, exposing employees to a 50 foot fall hazard.   D) Paper Mill west stairs to stores, on or about September 25, 2014, the top rail of the guardrail system on the platform at the top of the stairs was only 34 inches high, exposing employees to an approximately 20 foot fall hazard.   E) #2 Chipper, on or about August 5, 2014, the guardrail on the side of the step platform the employees used to cross over a piece of equipment was only 20 inches high, exposing employees to an estimated fall distance between 57 inches and 66 inches to a concrete floor below.  F) #1 Chipper, on or about August 5, 2014, the guardrail on the side of the step platform the employees used to cross over a piece of equipment was only 21 inches high, exposing employees to an estimated fall distance of 70 inches to a concrete floor below.

Runway #3, on or about July 24 , 2014, an employee operating a Hyster forklift traveled through a doorway (equipment doorway), where the operators vision was obstructed, without sounding the horn on the forklift or slowing down, exposing employees to a struck-by hazard.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:     A) Dozer Shed rear right wall, on or about August 7, 2014, an electrical duplex receptacle did not have a cover in place, exposing employees to a minor shock hazard.     B) Dozer Shed front left wall, on or about August 7, 2014, an electrical box containing electrical wires did not have a cover in place, exposing employees to a minor shock hazard.      Georgia Pacific was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.305(b)(2)(i) which was contained in OSHA inspection number 943256  citation number (1), item number (5), and was affirmed on May 5, 2014, with respect to a workplace located at 12551 Hwy 273 West, Cedar Springs, GA.
