29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a)  Pulp maintenance fab shop, on or about August 18, 2015, the gap between the adjustable top housing of the  bench grinder and the grinding wheel was greater than one quarter inch, exposing employees to a struck by hazard.    Georgia-Pacific Cedar Springs, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(b)(9), which was contained in OSHA inspection number 986883, citation number 1, item 005 and was affirmed as final order on February 10, 2015, with respect to a workplace located at 12551 Hwy 273 West, Cedar Springs, Georgia.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:)    a) Paper machine #1, west side wall in the basement, on or about August 18, 2015, the 480-volts electrical disconnect panel box had an unused opening on the right side from a missing knockout, exposing employees to electrical shock hazards.    Georgia-Pacific Cedar Springs, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 943256, citation number 1, item 004 and was affirmed as final order May 5, 2014, with respect to a workplace located at 12551 Hwy 273 West, Cedar Springs, Georgia.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a) Paper Machine #1, wet end, on or about August 18, 2015, the outer jacket on the 500-volts DC power cord connected to the transfer motor that runs the 2nd press bottom roll was withdrawn, exposing the inner conductors and terminals to strain.    Georgia-Pacific Cedar Springs, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(g)(2)(iii), which was contained in OSHA inspection number 943256, citation number 1, item 007 and was affirmed as final order on May 5, 2014, with respect to a workplace located at 12551 Hwy 273 West, Cedar Springs, Georgia.
