29 CFR 1910.272(j)(1): The employer did not develop and implement a written housekeeping program that established the frequency and the method(s) determined best to reduce accumulations of fugitive grain dust on ledges, floors, equipment, and other expose areas: a) Mid-Wood, Inc. located in North Baltimore, Ohio; On or about July 30, 2014 the employer failed to implement the company's written housekeeping program in that grain product spills were permitted to accumulate around grain elevator legs.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: a) Mid-Wood, Inc. located in North Baltimore, Ohio; On or about July 30, 2014 the employer failed to provide Appendix D of the respiratory protection standard to employees who wore filtering face piece respirators (dust masks) voluntarily.
