29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. Portage Valley Work Center; on or about 30 July 2014 and times prior thereto; where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. Employees routinely handle hazardous chemicals that require immediate first aid measures that include 15 minutes of continuous flushing of the eyes. This condition exposed employees to an eye trauma hazard.

29 CFR 1910.1200(f)(5): Chemical manufacturers, importers, or distributors did not ensure that each container of hazardous chemicals leaving the workplace was labeled, tagged, or marked in accordance with 29 CFR 1910.1200. Portage Valley Work Center, Recreation Cache area; on or about 30 July 2014 and times prior thereto; one 16 ounce and one 32 ounce spray bottles containing an unknown chemical substances were not labeled identifying the product and the hazard warnings associated with it. This condition exposed employees to chemical burns, poisoning, and skin dermatitis.
