29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times.  Warehouse, on or about 30 July 2014 and times prior thereto; the employer did not ensure that emergency exits lights were in working order. Three instances were noted:       a. In the maintenance bay on the North West side of the building, adjacent to emergency exit door; the left side of  the emergency exit lights was inoperative.        b. In the maintenance bay on the North West side of the building, adjacent to emergency eye wash station; the right side  of the emergency exit lights was inoperative.       c. South wall of the warehouse are; emergency exit lights were inoperative.  Condition exposed employees to delays in exiting the building causing smoke inhalation and fire hazards.

Warehouse, maintenance bay; on or about 30 July 2014 and times prior thereto; an air nozzle used for cleaning purposes exceeded 30 p.s.i. Air nozzle measured approximately 80 psi.  Air nozzle was not equipped with deflector to reduce the pressure of air flow.  This exposed employees to foreign objects being embedded to the body and eyes.
