29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired. Pisinemo Location #7: On or about June 24, 2014 and at times prior thereto, employees were using extension cords with a missing grounding pin to operate the Mitre Saw and the Rigid compressor for pneumatic tools exposing operators to electrical hazards.

29 CFR 1926.405(b)(1): Unused openings in cabinets, boxes, and fittings were not effectively closed: Pisinemo - Sellers # 7: On or about June 24, 2014 and at times prior thereto, the outside electric panel box providing power to contractors was not effectively closed. The knockout at the bottom panel was missing. Employees were handling the panel to tape and provide overspray protection and were exposed to potential electrical hazards.
