29 CFR 1926.50(c): In the absence of an infirmary, clinic, hospital, or physician, that was reasonably accessible in terms of time and distance to the worksite, which was available for the treatment of injured employees, a person who has a valid certificate in first-aid training from the U.S. Bureau of Mines, the American Red Cross, or equivalent training that can be verified by documentary evidence, was not available at the worksite to render first aid: Toka Homes 1 Project, Lot 15, Sells, AZ 85634: On or about 18 December 2014, the employer failed to ensure that a person with a valid first aid certification or equivalent training that could be verified by documentary evidence was onsite. Employees onsite that used saws and ladders were exposed to the hazards of being struck by the equipment and falls from heights.

29 CFR 1926.21(b)(2): The employer did not instruct employees in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: Toka Homes 1 Project, Lot 15, Sells, AZ 85634: The employer failed to provide an instructional program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage firefighting, exposing employees to the hazards of fire.
