29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Drill site: On or about July 30, 2014, and at times prior thereto, employees working on Rig #17 were exposed to trip hazards in two different locations. 1. The rig floor had a floor board located east of derrick which was arched/elevated approximately four inches. 2. The metal suite cases running parallel to the mud pumps had an open lid approximately three and a half inches.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Drill site: On or about July 30, 2014, and at times prior thereto, employees working at the end of the cat walk where the pipe wrangler is located did not have any rails. This condition exposed the employees to a fall greater than four feet.

29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: Drill site: On or about July 30, 2014, and at times prior thereto, the employee mixing mud at the hopper located west of the mud pit was not wearing the proper eye protection for the chemical being mixed. The SDS requires the use of safety goggles; the employee was wearing safety glasses which did not create a seal around the eyes.This condition exposed the employee to potential eye damage.

29 CFR 1910.134(k)(1)(iv): The employer did not ensure that each user could demonstrate knowledge of how to inspect, put on, remove, use, and check the seals of the respirator: Drill site: On or about July 30, 2014, and at times prior thereto, one employee mixing mud at the hopper located west of the mud pit did not don the respirator properly. The respirator has to be worn with one strap above the ear and one strap below to properly seal to the employees face. This condition exposed the employee to respiratory hazards.
