29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: Drilling site: On or about November 14, 2017, the surface landing to the stairway that led up to the rig floor was not flush to the floor; it was sticking up approximately 2-inches above the floor. This condition exposed employees to trip and fall hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Drilling site: On or about November 14, 2017, the employer did not ensure that listed or labeled electrical equipment was used in accordance with manufacturer's instructions. A microwave oven (1450-watts) and coffee pot (1200-watts) were plugged into a 15-Amp/1800-Watt Power-Tap. This condition exposed employees to electrical shock hazards.
