29 CFR 1910.28(b)(1)(i)(A): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a standard guardrail system: On or about November 14, 2017, there was an area that was missing a guardrail system on the rig floor on the back corner facing north. The open area measured approximately 36 inches in width and approximately 42 inches in height. This exposed employees to fall hazards greater than four feet.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points: On or about November 14, 2017, two electrical cables were showing some insulation crimping from being pressed on by the metal casing door. One of the cables powered the driller's console at 240V and the other powered the water tank at 120V. This exposed employees to electrical shock.
