29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o)(a): On or about July 28, 2014 and at times prior thereto, employees were working on Rig # 617. An inverter duty motor with pulley and belt system, which powered the high pressure washer, had its guard removed to be worked on. The employees were exposed to caught in between hazards.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and cables were run through doorways, windows, or similar openings: On or about July 28, 2014 and at times prior thereto, employees were working on a rig site, Rig #617. The electrical control house (VFD) had several cables which were being pinched by the door of the electrical panel box. This created electrical hazards for employees.

29 CFR 1910.305(j)(2)(iv): A receptacle installed in a wet or damp location was not suitable for the location: On or about July 28, 2014 and at times prior thereto, employees were working on a rig site, Rig #617. In the change house, there was an electrical outlet next to the sink which was not GFCI. Employees were exposed to electrical hazards.
