29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. On or about October 21, 2019, at the H&P International Drilling Co. Rig #423 in the Change house the electrical cord had a busted strain relief. The 120 volt cord provides lighting to the Parts Conex exposing employees to electrical/shock hazards.

29 CFR 1910.305(j)(2)(v): A receptacle installed outdoors in a location protected from the weather or in other damp locations shall have an enclosure for the receptacle that is weatherproof when the receptacle is covered: On or about October 21, 2019, at the H&P International Drilling Co. Rig #423 the outdoor receptacle (120V / 4outlet) located at the Rig Manager's trailer did not have a cover and was not protected from the weather, this condition exposed employees to electrical shock hazards.
