OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to anhydrous ammonia hazards:  (a) On or about 7/26/2014 - at Plant 2, 695 W Henderson Rd, Boaz, AL, employees are exposed to ammonia vapors due to the roll-up door between the machinery room and the maintenance shop with an aisle leading to the processing areas of the plant not being kept closed or tight-fitting especially when maintenance activities such as but not limited to filter changes are being performed.  (b) On or about 08/01/2014 - at Plant 2, 695 W Henderson Rd, Boaz, AL, employees are exposed to ammonia vapor hazards when exiting the machinery room and working immediately outside of the machinery room exit due to the pressure relief device discharge piping that relieves to atmosphere being located within about 15 feet of the personnel exit and not having access to emergency remote controls to stop the action of the refrigerating compressors outside of the machinery room.    (c) On or about 08/01/2014 - at Plant 2, 695 W Henderson Rd, Boaz, AL, employees are exposed to ammonia vapor and explosion hazards due to inadequate ventilation in the machinery room, the equipment in the machinery room did not conform to the requirements of a Class 1, Group D, Division 2 location, and no supervised alarm system is activated if the mechanical ventilation system fails or an ammonia leak detector is activated.  (d) On or about 08/01/2014 - at Plant 2, 695 W Henderson Rd, Boaz, AL, employees are exposed to chemical and frost burns due to an eye wash and body shower not being readily accessible outside of the machinery room.
