29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition. a) On or about August 6, 2014, in the facility, all places of employment and passageways were not kept clean and orderly. Materials including, but not limited to, production material and electrical equipment, had accumulated in work areas, thereby exposing employees to slip / trip / fall hazards.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines, whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a) Production Department - On or about August 6, 2014, employees were required to operate a Marvel Hacksaw, Model #9/F1, Serial #96532. The employer did not ensure that the entire cutting blade of the Marvel Hacksaw was completely guarded, thereby exposing employees to the hazards associated with exposed points of operations.
