29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolate: a) Shop Floor - On or about March 13, 2024, the employer failed to provide training to each authorized employee(s) performing service and/or maintenance activities on the Mach4 4020c Flow Waterjet machine to ensure the employee(s) acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. Employees were thereby exposed to machine hazards associated with not being trained as an authorized employee.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Shop Floor and in the Field - On or about March 13, 2024, the employer did not ensure that employee(s) who operate forklifts (Toyota Forklift, Model: 5FGC25, SN#: 5FGCU24-76006) were properly trained, evaluated and certified to perform the tasks in a safe manner, thereby exposing employees to struck-by hazards.
